Please contact me regarding my current fee schedule. Payment is expected upon services rendered and cash or checks are accepted.

A sliding scale is available on a case-by-case basis in the event of financial need.

While I do not handle insurance claims directly, I will provide an invoice containing all the necessary information to seek reimbursement. By submitting this invoice to your insurance carrier you may be eligible to get a portion reimbursed. I encourage you to look into the specifics of your health care plan.

Cancellations: At least 24 hours notice of cancellation is required. Less than 24 hours notice will result in being charged the full fee for the missed appointment.
